It is very important that we know where you are and have your current address and phone number. If you move house or change your telephone number, please contact us and let us know your new details as soon as possible. Without them we may not be able to contact you to highlight important stages in our surveys.
If you move outside the UK, please also let us know and tell us your address. We are not able to interview families living outside the UK but we can continue to send you findings from the study. If you move back to the UK, please let us know so we can invite you to re-join the study.

Even if you have decided not to take part in CNC anymore and have recently moved we would appreciate it if you could let us know to prevent us from trying to contact you unnecessarily at your old address.
